To become a new patient with RCP, call our office at 706-432-6866 and speak with one of our staff members. They will ask several questions to get you established with a therapist.
As part of establishing a professional relationship, we take pride in informing you of our services, policies, and expectations. Prior to your first appointment, you will be asked to read and sign the Patient Information Packet, which represents your understanding of, and agreement with, our terms of service. If you have any questions and/or require clarification, please be sure to ask us.
If the patient is a minor, the initial session will be primarily with the parent to gather history and learn about the issues. No child under the age of 8 may be left unaccompanied without an adult in the waiting room.
PLEASE ARRIVE AT LEAST 15 MINUTES EARLY FOR YOUR FIRST APPOINTMENT.
Unfortunately, HIPPA requires a lot of information to be completed. As a result, we have paperwork that is necessary to establish you as a patient in our office.
Your Patient Information Form includes the following:
- your demographic information
- your insurance information and your permission to file your insurance
- our office policies that affect your schedule of sessions
- how to reach us in emergency situations
- confidentiality policies
- your rights as a patient in our office
- our responsibilities as therapists
- our social media policy
- our cancellation policy
